Disability Services
Important Steps to Follow
1) Identify needs: A
person needs to identify themselves as someone with special needs to
the Office of Disability Services.
2) Contact Disability Services: An
appointment needs to be made at least 8 weeks prior to the beginning
of the quarter in which the student plans to enroll. This benefits
everyone's time line.
3) Bring, mail, or fax information:
The required documentation needs to be in the Disability Services
Office no later than 2 weeks prior to the appointment in order to
discuss eligibility for accommodations. Please be sure that all
information is marked as CONFIDENTIAL.
Some possible
types of documentation include:

multi-factor
evaluations (not IEP) that are no more than five years old
-
doctor's
statements (signed and typed on letterhead)
-
medical
records/hospital records (family physician, clinics, etc.)
-
psychological
reports
-
letters
of verification from the Bureau of Vocational Rehabilitation (BVR)
**Release of Information
forms are available in the Disability Services Office (central
campus) to assist in obtaining documentation.
4) Discussion of special
needs: if all paperwork is in order for the appointment,
discussion can proceed regarding the best method of equal access.
Decisions are made on a case-by-case basis.
5) Accessing
accommodations: The student will be responsible each quarter of
enrollment for:

giving a copy of their
class schedule to the Office of Disability Services prior to the
quarter of enrollment.

picking up, completing,
and returning paperwork within two weeks after the beginning of
the quarter.

if an individualized
placement test is necessary, the testing coordinator will
contact the student to set up a test time. Upon completion of
the test, students should follow the directions in the student
packet that reviews the guidelines on speaking with an advisor,
which involves registering for classes.

requests for
accommodations must be received at least 45 days prior to the
term for which they will be needed.
