Medical Insurance
Below is a list of the requirements that need to be met in order to receive the College's contribution to your H.S.A or H.R.A account:

All requirements must be met and you must provide documentation by May 31st of each coverage year. Please see below under Additional Resources for the Proof of Visit Form. Once completed, return the form to the Coordinator of Employee Benefits and Compensation, Crystal Howland.
