Unless noted as "Electronic", the forms are available as fillable PDF forms. After filling out the document, save the document to your computer, print, sign and submit to the Financial Aid Office via fax, email attachment or via U.S. mail.
Federal Financial Aid Forms
2021-2022 (Fall 2021, Spring 2022 and Summer 2022)
NOTE: Beginning July 13, 2021 and for the remainder of the 2021-22 academic year, the U.S. Department of Education (ED) is waiving verification of income, taxes paid, number in household and in college, except for identity/Statement of Educational Purpose and high school completion status in Verification Tracking Groups V4 and V5. Financial Aid applicants should check their MyRecords portal for required financial aid documents.
